DBDesigner

I have been using DB Designer for a while, but for some reason I hadn’t been using it to build join tables and foreign indexes automatically.
 

Because I am an idiot.  If you are using Ruby on Rails, you may also want to check out the dbmodel generator.  It will build your models from the DBDesigner XML file, including your has_many/belongs_to relationships.
